Accepted Forms of Payment/ Insurance Plans

  • Self Pay (Cash, Visa, Master Cards, American Express)
  • Our practice accepts most PPO plans from major insurance companies. Coverage varies by plan, network tier, and benefit structure, so we encourage all patients to verify their individual benefits directly with their insurance carrier.

PPO Plans We Accept

  1. Cigna, PPO including Open Access Plus, Local Plus, PPO with HSA, etc.

  2. Aetna PPO — Some Aetna PPO plans use limited or curated networks, sometimes labeled as Special Designation plans. If your card shows any special network, designation, or symbol indicating narrower access, please contact Aetna to confirm your benefits and notify our office in advance.

  3. Anthem Blue Cross PPO of California

  4. Blue Shield of California PPOWe are not contracted with the Blue Shield Tandem Network.

  5. UnitedHealthcare (Existing Patients Only) — We are not accepting new patients with UnitedHealthcare or its subsidiaries (Oxford Freedom, Surest, UMR, etc.). As a courtesy, we continue to see our established patients with UnitedHealthcare coverage.

  6. Blue Cross / Blue Shield Out‑of‑State PPO Policies — We are contracted with both insurers in California. If you have an out‑of‑state PPO plan, we are considered in‑network. Please bring your insurance card to every visit, as the alpha prefix often changes and determines where claims must be submitted.

  7. Medicare (Fee‑for‑Service)


Insurances We Do Not Accept

  1. HMO Policies (All Insurance Companies) We are not part of any IPA group, so we cannot accept any HMO insurance plans.

  2. Medi‑Cal and Medi‑Cal‑Related Plans Our Medi‑Cal patient panel is closed. We are unable to see patients with Medi‑Cal or any Medi‑Cal‑related coverage.

  3. United Healthcare PPO/HMO new patients


Understanding Your Coverage

Insurance benefits vary widely, even within the same company. Patients are responsible for confirming:

  • Whether our practice is in‑network for their specific plan

  • Whether the service is covered, partially covered, or not covered

  • Whether prior authorization is required

  • Whether deductibles, copays, or coinsurance apply

  • Whether their plan uses a limited network or special designation

Our team can help guide you, but your insurance company is the most accurate source of benefit information.


Self‑Pay Options

Patients who do not have insurance, whose insurance is not accepted, or who prefer not to use their insurance may choose to self‑pay for services. We offer:

  • Transparent, upfront pricing for office visits and procedures

  • Package pricing for certain evaluations or treatments

  • Flexible payment methods: As a self-paying option, we accept cash and all major credit cards.

Electronic Statements: We have transitioned to electronic statements to minimize paper use. Please ensure your email is added to your patient file, as this helps us avoid paper whenever possible.

Self‑pay patients receive a detailed receipt (superbill) upon request. It may take 1-2 weeks to receive, and they may submit it to their insurance for possible out‑of‑network reimbursement.

Dr. Preys has privileges to perform surgeries and deliveries at Cedars-Sinai Medical Center. She has been an active member of the Cedars-Sinai Team since 2001.

For smaller cases, she has privileges @ Prescision Ambulatory Surgery Center & 90210 Surgery Center, both located at
